Instructions For Sports Physicals Revised: 3/6/07
For each school year of participation, prior to any student participating in practices, inter-school practices, scrimmages, and/or contests, at any PIAA member school, the student is required to complete a physical evaluation for the current school year. A student completing a Comprehensive Initial Pre-Participation Physical Evaluation (CIPPE) during the current school year of participation need not have a re-certification for a period of twelve (12) months, unless the student suffers a serious illness or injury within those twelve (12) months.
Please note: Sports physicals must be dated after June 1st for each year of participation.
NOTE: All students participating in interscholastic sports must have had a tetanus shot within the last 10 years.
- Parent/Guardians must complete and sign sections #1, 2 and 3 of the PIAA Comprehensive Initial Pre-Participation Physical Evaluation form. Retain Section 5 & 6 to be used in the case your child participates in subsequent sports during the same school year. Forms are available in the nurse's office of the high school and middle school buildings.
- If you choose to use your family physician, please have the authorized medical examiner sign the section #4 and if the sport is wrestling, section 7 of the PIAA Comprehensive Initial Pre-Participation Physical Examination forms. If the student needs to be re-evaluated during the same school year due to an injury or illness, parents should take section 6 to the medical examiner to be completed.
Sports physicals dates are announced and held in the nurses's office of the high school and middle school buildings. Have your student pick up the forms from the nurse's office beforehand, sign and return the forms to the school nurse on or before the date of the scheduled sports exams held in the school. Dates for the after school physicals are posted on the website calendar for your convenience.
If you choose to have the physical done with your family physician, please take the forms with you. Completed forms should be handed to your child's head coach for the first day of practice.
Remember, without a current physical, the student/athlete
is ineligible to practice or participate in any sport.
Student/Athletes must have physical on file before the first day of practice.
