Instructions For Sports Physicals

For a student to participate in athletics, the student, as well as a parent, must have an account with Planet HS. (Directions on how to create an account can be found using the following link: https://studentcentral.bigteams.com/index.php?schoolID=44549&keyword=%2Fuser%2Fsignin&districtID=16494.) Once accounts are set up and "Athletic Forms" in those accounts are completed in PlanetHS, a physical may be done by a student's family physician or with our school's physician at the scheduled times throughout the year. "Section 7" of the CIPPE is the actual physical paperwork and, once completed, may be uploaded to the student or parent account. This form must be dated on or after June 1st of the current school year.

The process of completing the "Athletic Forms" is the same as mentioned in the paragraph above.

A student may not participate in any practice, scrimmage, or athletic contest at a PIAA member school, without completing Sections 1 - 7 of the PIAA CIPPE.
