Step 1: Registration Packet

Request a registration packet from Central Registrar's office at (570) 226-4557, extension 3000. Alternatively, you may download the entire packet or individual documents by using the links on this page. 

The following documents need to be completed and returned. You may either type the information directly on the form, print and return; or print the forms, handwrite the information, and return the registration information.

Step 2: Physical & Dental Examinations

Pennsylvania requires a physical examination at the time of original entry. Parents have the option of having that exam performed by their own physician and dentist. Private examination forms are due by the first week of October. Also, additional vision, hearing, and growth screenings will be completed during the school year.

Step 4: Gather Documentation

Copies of documents are acceptable:

  1. Birth Certificate (must be 5 years of age by August 31)
    OR Baptismal Certificate
    OR Passport
     
  2. Proof of Residency - The following are acceptable:
    - A deed
    - A lease
    - Current utility bill
    - Current credit card bill
    - Property tax bill
    - Vehicle registration
    - Driver's License
    - DOT identification card
     
  3. Immunization Records from physician 

    Certificate of Immunization / immunization exemption form

     
  4. Special Education Records (if applicable):
    - Individualized Education Program (IEP)
    - Evaluation Report (ER)
    - Notice of Recommended Educational Placement (NOREP)
    - Psychological / Psychiatric Reports

Step 5: Return Documents

Documents may be emailed, returned by mail, or dropped off in person at the District Office.

Step 6: Attend an Orientation Program

Kindergarten Orientation Programs are offered at our North Primary and South Elementary locations at the start of each new school year. Watch for announcements and mailing about the date and time of the Orientation Program.
