Athletic Physical Forms

Physical forms are available in the nurses' office for the student/athlete to pick up or can be downloaded and printed.  

Please read carefully the Instructions for Sports Physical page, failure to have the required paperwork completed and submitted restricts the student/athlete from practicing and participating in any program.

If your child is having the sports physical done in school, the parent/guardian areas must be completed beforehand, and your child may bring them to the nurse at the time of the physical or turn them in before the physical date.

If you are using your primary care physician-please take the forms with you for the scheduled exam.
